Community Smiles Dental also provides oral health education presentations and a take home oral health kit to youth at select area schools. Services we provide for adults
Community Smiles Dental provides dental treatment limited to comprehensive exams, dental cleanings, simple restorations, and simple extractions at our Menomonee Falls clinic. Please call 262-522-7645, ext. 2280, to discuss program eligibility. CSD’s services for adults are in conjunction with our MATC Dental Hygiene Partnership. New adult patients are accepted for the spring and fall semesters.
Adults who are experiencing pain or a dental emergency can be seen for a limited oral exam at our Waukesha Clinic or our Menomonee Falls Clinic. We have limited availability for emergency appointments each day. Please call 262-522-7645 to schedule an appointment.
At Community Smiles Dental, we have special programs for pregnant women and adults with special healthcare needs. Please call 262-522-7645 to discuss program eligibility.

How Do I know If I Qualify for Care?
Community Smiles Dental provides dental healthcare to individuals who meet these eligibility guidelines:
- Have BadgerCare/Forward Health/Medicaid insurance OR
- Have income that falls below 200% of the Federal Poverty Guidelines and who are uninsured.
For patients without dental insurance, a sliding fee scale applies.
A credit card or debit card for payment is preferred.
